Case Managers' SOAR Portal

SSI/SSDI Outreach, Access, and Recovery (SOAR) is a national program designed to increase access to the disability income benefit programs administered by the Social Security Administration (SSA) for eligible adults and children who are experiencing or at risk of homelessness and have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder. 

If you believe one of your clients is eligible for SOAR services, please utilize the resources below to start the referral and application process.

Referrals are accepted up to and until the SOAR team reaches case capacity. All referrals will be preliminarily reviewed at the monthly case conferences. If the form below is not currently available, the SOAR team has reached capacity. Please contact Lindsay Cutler, lindsay@rihomeless.org, with any questions.
